0277998152 2018-01-30 Production Wells No Violations This well was previously issued a compliance notice for nonproduction well shall be plugged by Inspector Cynthia Van Dyke on 05/31/2011. The inspection this day revealed that this well is equipped with a pumping unit, 4 1/2 inch diameter production casing, and production tubing. There are lines attached to the wellhead but it is difficult to tell if the valve is open for production. The tank battery for this well is a 100 barrel steel tank and, it sits about 30 feet from the well and needs to be moved to the required 50 feet from the wellhead. There was also no legible identification on the tank when I was there. There was oil on the ground inside containment and outside of containment that flowed into a low spot just below the wellhead. The area outside containment is in about a 6 foot wide by 2 feet tall area below the wellhead. At 1:25 PM I called Nathan Lane the on call Emergency Response inspector to report the amounts in. I had mentioned the nearby stream to him and walked down stream of where the spill had happened and saw no sign of sheen or staining in the stream. I then called Paul Valentine the owner of P&D Valentine at 2:07 PM and received no answer and left him a voicemail. Update on 1/31/18 I ran into Paul Valentine and he let me know he had received my call and he was going to send people to clean the spill up and move the tank as soon as he can.
0344652058 2018-03-29 Production Wells No Violations This well was issues a violation on 10/30/09 by Inspector Cynthia Van Dyke for well operation causing pollution and contamination, failure to legibly identify well, dike or pit not able to prevent escape of brine or other wastes, and violation of tank spacing requirements. The well was issued another violation on 05/31/11 by Inspector Cynthia Van Dyke for well shall be plugged and nonproduction; well shall be plugged or placed in temporary inactive status. The inspection this day revealed that this well is equipped with a scissor style pumping unit, 4 1/2 inch diameter production casing, and production tubing. The oil that was contaminating the soil has been removed from site and cleaned up. The tank has been moved, identification is legible on the 100 barrel steel tank, and a SPCC dike has been built around the tank. New yellow 2 inch plastic lines are running from the wellhead to the tank and fluid could be heard moving through the lines. The powerhouse to pump the well has a new motor on it. In RBDMS production for this well is up-to-date as of 2016 and shows 198 MCF of gas for 2016. This well is now in production and all violations have been resolved.
0396949528 2015-05-04 Production Wells No Violations Upon inspection, it was observed that this well is still noncompliant. The polish rod on the pumping unit is dry/oxidized indicating that the well has not been pumped recently. The well is capable of production as flow lines are attached to the well head and extend into the 100 bbl production tank. There is free oil product pooled up around the 6.625 surface casing, indicating a possible hole in the casing. The production tank is still too close to the well to meet ODNR DOGRM spacing requirements and there is contamination present around the tank bottom. The containment dike is compromised and would be ineffective in the event of a tank rupture/spill. No identification information is present on the location. The well has been historically noncompliant since at least 1/29/2004. A formal request for Chiefs Orders to plug and abandon this well will be compiled and submitted this week.

The shackle-line pumping unit is overgrown with brushy vegetation. There is old oil contamination around the well head and bottom of the production tank. 0960704434 2018-02-09 Production Wells No Violations This well was issued a compliance for well operation causing pollution and contamination, failure to legibly identify well, violation of tank spacing requirements, and dike or pit not able to prevent the escape of brine or other wastes by Inspector Cynthia Van Dyke on 10/30/09. On my previous inspection I found oil on the ground and inside the dike of this well. I spoke with Paul Valentine the owner of this well and gave him instructions to clean the oil up, move the tank so it is 50 feet from the well, and post proper identification on the tank or well. The inspection this day revealed that there is still a small amount of oil on the ground but most has been cleaned up. The tank has been moved 50 feet from the well, has a new earthen dike built around it, and there is proper identification on the well. There is still pollution on the ground and the compliance cannot be closed. I called Paul Valentine and left him a message about the oil I noted still on the ground and instructed him to clean the oil up and remove and contaminated soil.
1204353376 2014-12-29 Production Wells No Violations Upon inspection, it was observed that no effort has been made to bring this well into compliance. The well is idle, but is capable of production. The 100 bbl production tank needs to be moved to meet spacing requirements and an SPCC compliant dike must be built around it. I will contact Mr. Paul Valentine and inquire about plans to bring this well into production or to plug and abandon it.

Met with Supervisor and Paul Valentine, talked about what still needed done (clean up isolated patches of oil-soaked dirt, spread Gater over oily patches, move tank - when empty, IDs are posted) and discussed timeframe of 1 week. Post IDs, clean up oil spill to ODNRs satisfaction, place tank 50 from well, build dike around tank to hold 110% of volume. 4/22/10 Talked to Paul Valentine - he says tank is full, will haul oil next week and then have tank moved. 3/1/11 - Paul Valentine says he will have tank emptied and moved by end of month. 6/10- Tank still not moved, adding more violations.
1306870039 2011-05-31 Production Wells Violations Noted No change - remedy required by 11/09. Oil tank still not moved - about 35 away from jack. Trail to well very overgrown - no one has been to well in long time. Paul had commited to having tank moved by end of March, 2011. Annual production shows well has not produced since 2007. Will add additional violation at this time. Need to file for Temporary Inactive status. Need to move on to next step - pictures taken. 6/1 - called and talked to Mrs. Stonerock - suggested to her that proceed thru the courts to take the well back - Paul has no intention of acting responsibly and she said last check they received for the well was 2007.

Paul Valentine has not produced this well in many years and it was hoped that once/if lease was broke, then could proceed to get well plugged. I told Stonerock that I would continue to work with the help of our attorney, to get something done with the well.

1666124760 2009-07-27 Production Wells No Violations Oil spill found this day on Tom Stonerocks ground. Flow line to tank has come loose, has been floppying around in the air as well flows gas. Ground saturated with oil covering an area 100 x 200 around the tank and well. Kink in plastic line finally cut off flow and stopped further damage. Could have happened several months ago as owner not pumping well regularly. He was told to move tank several months ago and still has not done it. Clean-up progressing, starting with moving of tank off the side of hill. Oil travelled downhill about 75-100 but did not get in stream - travelled parallel to it. Mostly leaves saturated with oil - will be bagged up and removed. Probably more of a gas well than oil - just enough to make mess. Will continue to monitor. Spill report started - 3-4 bbl oil, an estimate.

1981388809 2004-01-29 Production Wells Violations Noted I was asked to inspection this well for the landowner - talked with him and he has never got royalty checking in 18? Years. Computer shows made 10 bbl/year in 1997. Inspection found old shackle-line jack incapable of producing - large tree has fallen on pumphouse, crushing it and making motor inaccessible. Tank on side of hill, no IDs, can see where water/oil has been drained downhill into run - but not in long time. Erosion ditch has been cut when draining. Have talked previously with operator regarding his Idle wells so will go ahead and give NOV to reinforce my words. Giving extra time due to winter conditions. 12/7/04 - Told Paul well was going for C.O.s - he says he wants to produce it - I said he had to get something done next week to prevent from going. 5/27/2005 working on well this day - should be producing next week.
