0692970845 | 2014-03-25 | Plug / Plug Back | No Violations | This day spent overseeing plugging operations by Berry Well Service. Plugging equipment was unloaded and set up, well head prepared to securely hold valve, location built. |
0861187441 | 2013-10-10 | Orphan | No Violations | On the day of my inspection, I witnessed an area (50x30) around the well that was saturated with oil, I noticed a smell of gas migrating from the well, I checked the area immediately around the well for natural gas with a Spherian gas meter which showed 0 level of H2S gas was not detected, Indian Camp Run Road is 40 south of the well. The oil from the well is contained onsite and did not enter the creek. |
1194439096 | 2013-11-15 | Orphan | No Violations | My inspection this day was with P.C. Lall, inspector for the federal Coast Guard program to show him the location to qualify for CG funds. Verbal approval was given. |
1381319243 | 2013-09-25 | Orphan | No Violations | Today I met the landowner/complaintant, Ted Korte, and DOGRM Inspector Mike Hartshorn to inspect the well involved in the complaint. This well had suddenly blown out on 9/11/2013, spraying oil over a 45x30 area, several buildings and some of the adjacent flower beds and grapevines. The well was believed plugged and when it blew out, a large (8 x 1) chunk of dirt was ejected from the well. This well is located 50 from Indian Camp Run which was not impacted by this event. The landowner has put up silt fence to protect Indian Camp Run from comtamination. The well contains 7.5 OD casing (estimated measurement, as the top of the casing is below ground level) and is still emitting a small amount of natural gas. It is located 14 from a livestock barn, 30 from an equipment shed and 50 from an artist workshop. The landowner lives across the road and gets his drinking water from a spring located behind this house. Information obtained from our database shows no water wells in the area. I am recommending this well be considered an emergency due to the oil and natural gas being expelled from the well into the surrounding environment and potential into the nearby stream. There will be overhead electric lines to contend with when accessing the well site. The dirt access road that runs around the backside of the livestock barn should be used to access the well. There are no records available for this well and a permit number will be assigned. Inspector Hartshorn indicated that, based on his knowledge of other wells in this area, this may be a Berea well with an estimated total depth of 1100. Pictures taken, distances measured and meter readings taken. SEM is 207. |
1381400544 | 2013-10-09 | Orphan | No Violations | Todays inspection is the second at this site. I found that the fluid level in the well had gone down and I could see that there is a partial bridge about 4 down the hole with 2 strings of tubing visible coming out of the bridge. The top of the tubing is at the same level as the top of the bridge. Both strings of tubing appear to be the same size (1.5 - 2) and both are bent which will make retrieval more difficult. When a stone is dropped down into the tubing it hit fluid. Gas was still venting from the well but was minimal (less than on 1st visit). Well has not blown out again and landowner has done some dirt work to make assess to the well easier and drier. Pictures taken. Research on surrounding Berea wells tells me the depth is more accurately 1050-1100 |
1389189241 | 2014-01-08 | Administrative Inspection | No Violations | I talked to Bill Mitten/Knox Energy and requested an estimate for the T. Korte well. He said would e-mail it to Supervisor G. Chini today. |
1396297804 | 2014-03-31 | Plug / Plug Back | No Violations | Well plugging is finished this day. This well was plugged solid from bottom to top. I will confirm cement up to surface tomorrow, and then have the well identification installed. |
1402241245 | 2013-11-21 | Orphan | No Violations | Measured casing in hole - found it to be 10.5 ID. |
1410979516 | 2014-04-14 | Final Restoration | No Violations | My inspection this day is to approve the reclamation work completed by the plugging company. I found that all equipment has been removed, the location and access road have been finish-graded, seeded and mulched and the grass is beginning to grow. The landowner has agreed that the work is satisfactory. |
1916363966 | 2014-03-24 | Administrative Inspection | No Violations | I talked with Bill Mitton of Knox Energy and they will move the rig and equipment out on location to plug this Orphan well tomorrow morning, nipple up and start work on the hole late tomorrow. He will call me in the morning with more information. |
2063409318 | 2014-03-26 | Plug / Plug Back | No Violations |
Rig crew prepared the well for plugging. They brought out the overshot tool and grabbed onto broken tubing. The water truck filled tank with 80 bbl saltwater. Last 2 hours of the day spent at courthouse researching records. |
