I found that preparations were underway, but no rig equipment was on site. Some secondary containment was staged, and a number of frac tanks and light plants were in place. I took GPS readings for the well and access road entry. A security guard was stationed along the access road.

There are concrete jersey barriers protecting the wellhead and production equipment from vehicles. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.

0595409290 2015-12-28 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of four, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1676408366 2013-09-11 Casing String Witnessed No Violations

-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The cement slurries were mudscaled to verify the target density. Schlumberger pumped 152 bbls of lead cement at 13.5 ppg, and 675 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 383 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 12:30am on 09/12/2013. The plug landing pressure was 2828 psi, and it was raised to 3380 psi, and held for 5 minutes. Pressure after 5 minutes was 3385 psi. The float held when the pressure was released, and 3.5 bbls of water bled back. Approximately 25 bbls of the Mudpush spacer returned to surface.
