1566938426 | 2015-01-24 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 13.375 surface casing. Nomac Rig #71 spudded the surface hole on 1/23/15 and drilled a 17.5 hole to a depth of 636. Wyoming Casing rigged up and ran casing to a depth of 628. The rig was losing circulation (municipal water) of approximately 20 barrels per hour at a depth of 170. Schlumberger rigged up the cement head, tested the lines, and pumped 25 barrels of gel and 10 barrels of water ahead of the slurry. They then mixed and pumped 126 barrels of cement slurry, dropped the plug, and displaced the plug to the collar with 90.4 barrels of water. The plug landed at 316psi and the pressure was increased and held for 5 minutes at 975psi. The pressure was released and the floats held with 0.25 barrels flowing back to the truck. 28 barrels of cement circulated to surface. There were no circulation issues during the job and the cement held at surface. See casing ticket for more details on this job. No violations. |
1875471865 | 2014-10-22 | Administrative Inspection | No Violations | I was on location with Chesapeake representatives Steven Rybicki, Matt Garrison, Seth Wehr, and landowner Dean Ott to conduct Pre-Construction site inspection #s APATT026047 and APATT026051 for the Dean Ott Pad and the Donna Ott Pad. See Pre-Site inspection reports for site specific details on construction. |
2021917913 | 2015-11-10 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed. |
