0245086100 2015-11-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.

0324728713 2015-03-19 Casing String Witnessed No Violations I was on location to witness the cementing of the conductor casing. Top Hole Drilling augured a 26 hole to a depth of 120 measured from ground level (GL). 20 conductor casing was butt welded and set on bottom with holes cut at 3 and 6 off bottom to allow circulation. A 20 steel plate with a 2 ball valve was welded to the top of the casing. Express Energy Services rigged up to the 2 valve, tested the lines, and pumped 6 barrels of water to break circulation. They then mixed and pumped 48 barrels of cement slurry and displaced the slurry to 110 with 39 barrels of water. 12 barrels of cement circulated to the cellar. No violations. 0838339603 2015-03-30 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #71. They are currently drilling the 8.75 vertical section at a depth of 5,500. The location is in good condition and all drilling equipment is on containment. No violations.
1491894412 2015-03-27 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac Rig #71 drilled a 17.5 hole to a depth of 962. The hole was logged by Baker-Hughes and Wyoming Casing rigged up and ran 13.375 surface casing to a depth of 952. Schlumberger rigged up the cement head, tested the lines, and pumped 25 barrels of gel and 10 barrels of water ahead of the slurry. They then mixed and pumped 190 barrels of cement slurry, dropped the plug, and displaced the plug to the collar with 140 barrels of water. The plug landed at 400psi and the pressure was increased and held for 5 minutes at 900psi. The pressure was released and the floats held with 0.5 barrels flowing back to the truck. 50 barrels of cement circulated to surface. No violations.
1866603858 2015-03-25 Drill / Deepen / Reopen No Violations I was on location to check the status of Nomac #71. They began moving equipment from the Dean Ott pad on 3/23/15 and are currently 100% moved and 75% rigged up. They are hoping to spud the surface hole tomorrow afternoon. The location is in good condition and all equipment is on containment. No violations.
