0618977971 2017-06-29 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter production casing, and 1 1/2 inch diameter production tubbing. The well was written up for contamination by Supervisor Eric Conrad. While I was on location I could see there was new soil where the contamination has been cleaned up. The power pole and tanks have been removed from location. All contamination has been removed from location. This well is now in compliance.
0969493251 2018-09-24 Production Wells Violations Noted On this day, I am conducting an inspection per request of Southwest Supervisor Eric Conrad. Frederick Petroleum (owner #6009), has failed to maintain liability insurance (Chiefs Order 2017-24). The well is equipped with 4.5 inch diameter casing, 2 inch diameter tubing, rods, and a pumping unit with an electric motor. The well appears capable of production. The casing valve is closed, and the tubing valve is open to a plastic flowline running into a polymer tank near the wellhead. There is (1) 1,000 gallon polymer tank and a welded steel cube storage tank that measures 6 x 6 x 6. There are no flowlines attached to the steel tank. Additionally, the drain valve to the polymer tank is open with stressed vegetation approximately 4 feet wide and 20 feet long, running downhill into a wooded valley. The area was tested with a Low Range Quantab with results of 300 parts per million. It will be noted that the fluid sample was tested during a rain event. Production records are current with 0s reported from 2015 - 2017. I spoke with Mr. Tom Morrison of Frederick Petroleum on 9/18/18, and he indicated that he currently owns the wells and is aware that his liability insurance has lapsed. Pictures were taken. The contaminated soil will need to be removed and disposed of properly, establish new vegetation, place identification at the wellhead, and adjust the tanks to meet the minimum required setback spacing of 50 feet from the wellhead.

1544535911 2018-12-11 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. No identification has been posted and the tanks have not been adjusted to meet the minimum setback distance from the wellhead. There does not appear to be an attempt to address the residually contaminated soil at the base of the polymer tank. No new pollution and contamination appears to have been released. It will be noted that its 20 degrees Fahrenheit, and the ground is frozen around the tank. The well is equipped with 4.5 inch diameter casing with the valve closed, and 2 inch tubing with the valve is open. A flowline runs from the tubing to a 1,000 gallon polymer tank. There is a steel cube storage tank, approximately 35 barrels, but no lines run from the wellhead to the tank. A drain valve at the base of the storage tank is closed with a 2 inch plastic line attached, and placed on the downhill slope behind the tank. The well is setup to be produced by means of a pumping unit with rods and an electric motor. I spoke with Mr. Tom Morrison on 12/11/2018, and he stated that the well is not pumping, there has been no new release of fluid, and that he is in the process of selling the land and the well. He also indicated that there is oil inside the steel tank, and that he plans to remove both tanks from the property. Updated pictures were taken.

1794533343 2017-11-17 Production Wells No Violations This well was not in production. I spoke with the well owner who confirmed his wells were shut in and not producing, because he couldnt afford the insurance.
1819980462 2016-07-15 Production Wells Violations Noted My inspection today finds the well is equipped with a pumping unit that is powered by an electric motor. An active electric service meter is located on a pole forty feet from the wellhead. A one inch pipeline is running from the casing head towards a dwelling with the valves open. A one inch plastic pipeline runs from the pumping tee to a water separator (knockout) on top of a fifty barrel steel tank. A small amount of oil was standing at the base of the fifty barrel tank. An area 10 by 4 of brine affected soil is located to the Northwest of the base of the tank. No identification could be found at the location. The landowner and well owner Tom Morrison arrived on location and informed me that the compliant was levied by a tenant that he recently evicted from the property. Mr. Morrison stated that the well is used for domestic gas. I informed Mr. Morrison of the violation for failure to legibly identify the well 1501:9-9-05(A)10 and gave him until 8/01/2016 to place identification at the well site and clean the area affected by the oil and brine.
2058758367 2019-04-09 Production Wells No Violations On this day, I am conducting a compliance notice follow-up and order-follow-up inspection. Frederick Petroleum (owner #6009), has failed to maintain liability insurance (Chiefs Order 2017-24). No progress has been made to gain compliance. The well is equipped with 4.5 inch diameter casing and 2 inch diameter tubing. There is a low pressure regulator present on the 4.5 inch casing with the valve closed, and a plastic flowline running into the polymer tank from the tubing with the valve open. It is setup to be produced by means of a pumping unit with rods and an electric motor. Vines are grown through the drive wheel on the unit. The polymer tank and steel cube tank still reside within close proximity to the well. A drain line running downslope from the base of the steel cube tank is present with the valve closed. No new pollution and contamination is witnessed. No identification is present at the tank or wellhead. Updated pictures were taken.
