Bond log run and discovered hole had caved in all the way up to the bottom of the surface casing. Perfed and squeezed Lime plug and Berea plug at 2160 and 948. Shut down after the Berea plug was set.

635447982 2011-09-13 Witnessed plugging operations. Crew tried to achieve circulation through 4.5" casing to set Berea plug. Circulation was achieved too quickly, and tubing had to be ran to set Berea plug through perforations. When pumping cement, circulation was achieved on the backside of the 4.5" casing. After plug was pumped, crew pulled tubing then shut down so that plug could be tagged the next day.
