1310389535 2011-07-08 The Division received a complaint alleging idle well. An inspection revealed idle well consisting of 4.5" casing with a swage, valve and 8" wellhead. A 2" steel flowline near the wellhead was not connected. No house gas is received from this well. A review of RBDMS records showed that 2001 was last year for commercial production. Verified no reported production for 2010 with Central Office. Identification was not observed at the well or tank battery. The shared tank battery consisted of (2) 100 bbl tanks, separator, drip and meter. A GPS reading was obtained for the well and tank battery locations. On 5/5/2011 and 7/6/2011, the well owner was verbally instructed to plug or produce wells on this lease prior to the issuance of this Compliance Notice. The well owner shall plug or produce this well. If said well is placed back into production, identification is required for both the well and tank battery(shared) locations. Provide identification at the tank battery to show permit number, lease name, well number, county, owner name, and emergency contact number. For multiple wells produced into common tank(s),the well shall also be identified as to owner name, permit number, well number, and lease name. NONEC
1323141741 2014-08-05 This inspection was conducted as a final restoration status check. The well has been plugged. Equipment has been removed and the location has been restored. Final restoration meets ORC 1509.072 requirements.
