-1247558904 2007-06-04 FOLLOW UP TO NOV # -1495209655 FOLLOW UP TO NOV # -557564005 I inspected the well location with Jeff Fry. The annular disposal lines seem to be disconnected although it was hard to tell as the lines are buried. The well owner needs to disconnect lines and insert plugs. There is also a mess around the tank battery by the road. It appears that the brine has run over and the dike has not contained all of the fluid. There are several nearby pine trees that have recently died. It appears that there is either a leak in the line or the dike. I called Dick Poling and he was aware of the leak and is taking care of it. The pine trees are going to be replaced, the tanks battery is going to be moved back off the road another 100 feet, and the AD lines will be plugged at the well head. Annular disposal is currently not hooked up.
-1254310658 2012-05-21 An inspection of this well was requested by Central Office to verify Annular Disposal (AD) connection. During the inspection, it was noted that the AD remains disconnected. The tank battery consisted of (1) 200 bbl tank and a separator.

1486131990 2013-10-17 Inspect well. Well was equipped with a pumping unit and motor. It was not pumping during inspection. The tank battery consists of (1) 210 barrel tank and gas separator. No identification was posted at the well or tank. 9-9-05(A)10
