The soil around the area at the well head is still contaminated. I spoke with the field supervisor at the well site and informed him the area around the well still need addressed.

A 1" black plastic flow line running from the well to the tank had been severed in the past and released oil onto the surface of the ground. A trench had been dug to burry the line, repairs had been made to stop the leak and straw had been placed on the surface but oil and contaminated soils remained at the surface. Oil had spread over the surface at the site of the break contaminating an area of roughly 10 x 10' then moved through the trench down hill towards the road roughly 175 feet. Dike shall be emptied of fluids and the cut in the dike shall be repaired, or dike shall be replaced. Any tanks in the dike shall be equipped with locks or plugs. Stuffing box shall be repaired or replaced. All lines used in the production of oil and gas shall be buried at least 24 " below the surface. All oil, contaminated soils, and oily materials shall be removed and disposed of properly. Steps shall be taken in order to encourage vegetative cover in the affected areas. Any affected areas shall have top soil replaced where necessary and be reseeded and mulched. 9-10-05

1752574468 2013-03-08 Follow up to 1362601654. Water still in dike but the cut had been partially repaired with a shovel. No plugs in the tanks. There is still contaminated materials on the surface. After speaking with the landowner she pointed out to me the settling tank for there developed spring was less than 40' from the contaminated soils. The developed spring consisted of a 15' long piece of schedule 40 PVC that level inside the settling tank was less than a foot below the surface. From there the water flowed into a cistern near the house. After looking in the settling tank I did not see any immediately visible contamination. Pictures taken. Settling tank location = 39.43208 -81.91226. I immediately called Kyle Baldwin to arrange an inspection with our hydro geologist. I notified the company of the proximity to the families water supply and the need to clean the site immediately.
