I received a phone call from Marlene Hall at Uniontown office at 9:13AM this morning about a complaint from Ms. Caroline Caldwell at 14737 Garfield road, work phone number, 330-332-1017. She stated her gas to her water heater at quit. The well was considered idle. I called Ms. Caldwell at 9:18AM. She stated the same thing. That her gas from the well had quit coming into the house. I proceeded to the well site. GPS'd the well. Then I called Mr. Steve Svetlak. He did not know the well had quit producing. He stated he could swab well and get gas to come back. I called Ms. Caldwell back and told her about the swabbing. I gave her Mr. Svetlak's phone number. I also told Mr. Svetlak about her phone number. I will follow up on how the phone conversation went. I did tell Mr. Svetlak that the well in question needs to be either put back into production or plugged. He wanted it back into production. I GPS' d tank battery. Case continues.

I inspected the well to see if a rig had been on the site. It had not. I called Steve Svetlak(330-280-3167) He stated that he was just getting ready to call me. He had checked the well pressure on the past Friday, 1/25/08. The wellhead had 580psi on it. Someone had shut the ball valve off to the house. I asked if he would call Mrs. Caldwell and tell her. He would call and tell her what she needed to do to get the gas to her house back. I called Mrs. Caldwell later in the day and she said that Steve had called her about what needs to be done and what it would take to start getting gas to the house again. She thanked me for all the help that was done. Case resolved.

On 3/15/ 2013, I was notified by the Uniontown Office of a compliant filed by Carolyn Caldwell about the above well. She stated that the mineral rights had been awarded by the courts back to the property owner (Carolyn Caldwell). The last recorded production was in 2000. She used some house gas in 2009. On 3/18/2013, I performed a status check on this location. There is no production unit or tanks on her property. All valves are shut at the wellhead. No ID signs are posted at the wellhead. Well incapable of production. See copy of Court Affidavit of Forfeiture recorded in Mahoning County, Ohio. Well should be plugged.
