The tank battery has been moved to the east of Dorfman's tank battery. I GPS's the new tank battery site and the wellhead. All the debris has been removed. Seed has been sewn around the new tank battery. This inspection satisfies 1501:9-9-05. There were no violations at this time.

I conducted a status check on this site and found that the well is equipped with a pumping unit and connects to a tank battery that consists of one 200 barrel stock tank, one separator, and an earthen dike. There was no brine storage tank present. Proper identification was posted and I found no leaks or spills at the well head or tank battery. No violations at this time.

I performed a random field inspection. I GPS'd both the well and the tank battery for the first time. The tank battery area inside the dike had oil leaking around a 2" tee. A 3"X 2" swedge was leaking around the top of the stock tank. The dike was cut in some areas and there was no dike in other areas. There was no ID sign. I called Mitch Fitch from Enervest, (330-495-3962). He would go look at the site. Mitch called me later and agreed with everything that I had found. He will have it cleaned up within a week. I told him that I would check on it this Monday, 9/8/08.

I received a call from Tom Basick(330-575-9248) about a stock tank blowing up at this tank battery. There were 2 400 barrel stock tanks. They had a total of 50 barrels in them. On 10/30/10 some unknown cause ignited the one stock tank and launch it across the road some 320 feet away. The Beloit Fire Dept was on the scene letting the fire burn itself out. All the oil was burned up in the fire. Barnett and Son Excavating was on the scene cleaning up the mess. The other 400 barrel stock tank did not ignite. But the bottom swelled out. The Beloit Fire Chief, Mr. Barrett had no direct cause for the explosion of the stock tank. The clean up continues.

I performed a status check on the tank battery installation. The other 400 barrel tank has been removed as will as the ladder, etc. I called Tom Basick for a status check(330-575-9248). Tom stated that the tank battery will be moved to the west of Dorfman's tank battery to get away from the electric lines. I will check back later.

I performed a NOV follow-up. The 2" tee line was repaired as was the 3"X2" swedge at the top of the stock tank. Both were leaking before. The dike area was built up. The ID sign was posted. This NOV is resolved.
