Lease road gate and lease road remain as well as 2nd gate at the turn to where the well was. A tree has grown up in front of the gate, gate can no longer be closed. There is a fence that runs parallel to the lease road, gate appears to be part of that fence. Debris from the north side of the gate was laying in the grass. Contacted John Flemming requested that the gate debris be removed gate be repaired or removed. Once land owner waivers have be/.en submitted for the lease road and the gates, restoration will be approved.

New chart in separator date on chart was 09/17/2008, no production shown on the chart. Tank battery has been cleaned up and lease road has been mowed. Area around well head has been mowed. Dike around tank battery is in good repair. Sign on tank battery needs repaired. Called John Flemming from company for status of the well. No production has been reported since 2000.

Lease road is stable and in good repair. Chart was put into separator in September of 2008. No production shown on chart. Valves are closed at the wellhead. Well is leaking around the 2.5 inch pipe. Called John Flemming, they are planning on plugging well. Informed John that I need to see a permit to plug within two weeks, or enforcement action will be taken.

Bill Fitzmartin from Range Resources on site at the time of inspection. Bill was making repairs to the well. Well is going to be put back into production, once repair parts are put into place.

On site for plug job. Operator set bottom plug through tubing. Operator pulled tubing, ripped 4.5" at 2955'. Pulled 4.5" joints to set lime plug. Operator attempted to break circulation, operator could not obtain circulation. Operator ran out of fluid, I requested lose circulation material be brought on site next day. Circulation needs to be obtained before lime plug is set. When operator shut down hole went on vacuum.

Site has been restored. Road remains, there are ruins of an old home in the woods near the tank battery location. Road was pre-existing. Fence has been repaired.
