An inspection of this well was conducted with the owner, Mr. Gary Nethers to verify correct location prior to plugging and abandonment. During this inspection, it was noted that dirt had been excavated around the wellbore, evidence of old clay also existed; could hear fluid from most likely artesian water near surface of drive pipe; the fluid was down approx 2 seconds. Mr. Nethers stated that they excavated the dirt around the wellbore after landowner told them about location. Further stating the casing had been level with the ground and never cut off. Initially, Mr. Nethers could not remember plugging said well. The well location was also verified Central Office and a GPS reading documented from this inspection. RBDMS records show well owned by Gary Nethers and plug permit issued on 10/1/1992. Currently, no plug report was ever documented. Mr. Nethers was instructed to run tools in hole, verify obstructions, and notarize plug permit. Note: Mr. Nethers had been unsure of permit numbers therefore, filed and received 2012 Plug and Abandon permit for this well. Mr. Nethers provided a clay ticket from 12/15/1992 referencing 13 tons for the "Moran" lease believed to be for this well. A second ticket that he provided also referenced the "Moran" lease, but the date of 10/12/1989 matches the "dry hole" plug and abandon for Permit No. 5370.
