-693532978 2013-12-12 Landowner had complained of a smell of gas. I met with Jeff Burke of Rainbow Oil & Gas at the well site. Owner was venting gas into the 100 Barrel steel tank and allowing it to be released from the manhole in the tank. Owner stated the lines had not yet been laid due to right of way issues with the adjacent landowners. He stated work laying the line would begin early next week. All gas vented to the atmosphere must be flared, with the exception of gas released by a properly functioning relief device and gas released by controlled venting for testing, blowing down and cleaning out wells. Unlawful venting shall cease immediately and releif devise or flare must be installed if venting is to continue. 9-9-05(B)

1374594658 2013-07-22 Witnessed surface cement job. 3 centralizers were ran at 64', 224', and at 352'. 417 feet of new 7" surface casing was set in 430 feet of 9 7/8" hole with 125 sacks of Class A cement with 1% gel, 100% excess used. Hole conditioned and good circulation established before cement pumped. During cement mix the pressures at the pump truck reached 500 psi and Formation cement shut down truck. Pressure was bled off the lines and the pipe was pulled a few feet out of the hole. Lines reconnected and circulation established immediately, continued on with job. The pressures reached 500 psi and the pipe began to move up out of the hole again, before the displacement was run. Again the pressure was released and the pipe pulled, now 10-15 feet out of hole. The job was restarted and cement returns were seen at the surface, before the displacement was ran, with less than 1 Bbl of returns. Pressured up for the third time and the well was shut in with casing full of cement. Company plans to cut off surface casing at ground level and drill out cement.
