My inspection found the well to be equipped with a plunger lift unit. The annulus valve is closed. The demco valve and the valves leading to the flow line are closed. The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The tank battery is over grown with weeds. The proper identification is on the tank battery. The lease road is in stable condition. The last reported year of production was 1999. I will contact Mr. McDaniel, a supervisor for Enervest about the well. The well shall be placed back into production in 90 days. If the well is not producible a plugging permit shall be obtained within 90 days and the well shall be plugged within 180 days of this compliance action.

I was on location to witness the plugging of this oil and gas well. Nick's Well Plugging is the contractor doing the plug job for Enervest. Nick's rigged up, and set the Clinton plug through the 1.5" tubing with 30 sacks of Class A Cement and 50 pounds of cottonseed hulls. The plug was displaced and the well was shut-in until the following morning. No violations.

I was on location to witness the remainder of this plug job. Nick's Well Plugging set the Lime plug and the Surface plug with my permission without being present. I was on location to witness the top plug. Nicks rigged up and broke circulation and set 62 sacks of Class A cement to the top of the hole. Cement circulated to surface. The remaining 5 joints of 4.5" casing was removed from the well and the hole was topped off with cement. The GPS Coordinates are +/-7'.
