An area around the wellhead roughly 180' x 75' is void of vegetative cover. There is significant erosion to along the lease road as well as sediment entering a creek to the west of the lease road. Photos taken. The owner or the owner's agent shall grade or terrace and plant, seed, or sod the area disturbed that is not required in production of the well where necessary to bind the soil and prevent substantial erosion and sedimentation. Failure to comply with in the given time period will result in a material and substantial violation. NONE5
-1238225063 2012-10-11 Positive drainage has been restored along the side of the well site. The lease road has been smoothed out and water bars have been placed to divert drainage away from lease road. The entire are has been seeded and mulched. Straw bails have been placed to stop sedimentation in the creek. I will return on the due date to ensure vegetative growth has been established.

The 4.5 production casing is hooked up for annular disposal. The tank battery is equipped with a 100 barrel storage tank and a separator. The drain in the dike is not equipped with a shut-off valve.
