-380108228 2009-08-17 Driller depth 3201', drilling on fluid. Pits are in good repair. Small amount of Diesel fuel spilled on ground, quart or less. Operator was mixing topsoil with spill to cover it up. Asked driller to get peat sorb on site for spills.
1250179126 2009-08-12 On site for surface cement. Jay Cheslock DMRM supervisor waived the CND for the well. Berea was shallow, bedrock was Berea. Cement job went well, 4 BBL return of cement to surface. Universal the service company on site had pump issues. Not able to completely displace cement. Service company flushed lines attempted to displace 2nd time. Unable to displace. Service company wanted to try a third time. Driller and myself agreed best to drill out cement than risk damaging surface job. Third attempt not made. Cement held at surface, hole was static.
1259783687 2009-12-02 Well has been put into production today. Annulus pressure was 40psi. Site has been ruff graded. There are significant slopes to the east, north and west of the site. There are intermittent streams in the area. Sent an e-mail to Tim Reily from John D Oil requesting silt fence be installed. There is a stockpile of wood chips on location, suggested that the wood chips could be used as mulch to prevent offsite erosion. Lease road is stable and in good repair. Pits have been closed. Permit identification sign is posted at the end of the lease road.

1374237420 2013-07-19 My inspection found the well to be equipped with a plunger lift unit. The annulus valve is closed. The well and tank battery are in good repair. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
