-1200468408 2006-08-03 Complaint called in to Cambridge Office by Allen Hiv nor of well owner suspected of digging hole on property and disposing of waste out out water knock out tank, inspected site and found no evidence of any wrong doings area was seeded and mulched upon my arrival trace of oil where tank was located,

landowner called @9:10pm and wanted too know results of my inspection told him that I found no evidence of any contamination he hung the phone on me apparently he did'nt care for my results.

-2092523734 2013-01-28 I met with Allen Hivnor at the tank battery location where he told me the tank was leaking from the bottom. The well tender, Jim Welch, was also present and he confirmed this issue. Jim stated the line to the leaking tank was shut off and the oil was removed on Sunday. He was going to remove the remaining brine to the adjacent tank on Tuesday. I witnessed him measure the depth of 1' 3" as the level of brine in the tank. There was no visible contamination in dike when I arrived, but I told the well tender that the tank and contamination have to be removed as soon as the ground dries out. I also suggested he rebuild both dikes when they have the equipment at location. Josh Garland accompanied me to the location and we Quantab tested the water just outside of the dike and did not find any issues of contamination. I also called Joe Altier and have yet to receive a return call. 9-9-05(A)8
-277534788 2008-05-02 Restoration work at site satisfactory, well is equipped with pumping unit and 1-100 bbl tank with dike in place,also landowner was concerned about leaking stuffing box on pumping unit called and requested owner to replace packing he stated he would have well tender replace it 5/2/08.
1359733252 2013-01-31 Compliance Action Follow-Up #2092523734: Joe Altier called me this morning and told me he will be out there to clean remainder of brine from the tank. He stated however he cannot remove the tank until the ground has dried because the heavy equipment would tear up the area. He plans to remove the tank and replace it with another. He stated he was not sure when he would be able to remove the tank. He stated he would also remove any pollution and contamination within the dike once the tank has been removed from location.

1951297600 2013-05-17 Joe has replaced the tank even though the dike is in fair condition, I have asked him to build up the sides. I also asked Joe to get a bull plug on the new tank. There are no other issues to report at this time.
760635049 2013-05-15 Joe Altier called this morning and said the contractor should be on location tomorrow to finish the tank and dike issues. I went to location today and saw the replacement tank and called Joe. He said as long as it does not rain they will replace the tank tomorrow around 900am. 5/16/2013: Joe called and said the tank was replaced at 12:45pm.
