-1175622590 2013-05-28 I met Jon Scott to collect brine samples from the 1000-gallon poly tank which stored and eventually disposed of fluids into an illegal annular disposal process into this well. Collected samples for Group Oil and Gas Analysis, Field Sample ID #276 AD Connection. Jon constructed a water dipper sampling device to collect the samples. The unfiltered samples were decanted into two different size containers, a large one liter/one quart plastic cubic container and the other a 250-ml/8-ounce poly bottle containing HNO3 preservative.
-456813789 2013-05-23 Inspector Morgan Button inspected this well on 2-20-13 and found an annular disposal connection. I was called to verify the connection and to determine if the annular disposal is approved. My inspection revealed a production well with a pumping unit connected to a mechanical separator (5 feet from the wellhead) with a flow line to a 1000 gallon poly tank. The poly tank drain valve was open with a 1" black plastic line connected to the annulus (annulus valve was open). Annulus connection 1" plastic to 1" black iron 90 ELL, a 1" steel nipple, a 1" by 2" bushing, a 2" ball valve (open). There were no check valves on the line from the poly tank to the annulus. Fluid level in the poly tank was approximately 8". A search through the well records revealed no approved annular disposal. violation was issued by Morgan Button MRI 2.
-630888042 2013-05-24 I called Craig Ross (Equity Oil & Gas Funds Contractor) At 11:03 am. I told Craig to shut the valves (brine tank drain and annular valves) and disconnect the annular disposal line. Craig stated that he will disconnect the annular disposal line as soon as he can. He said it is possible that it could be done on Saturday 5-25-13 or at least by Wednesday 5-29-13. I told him the work needs to be done immediately per an e-mail from Tom Tomastik UIC section.
1006812472 2013-05-20 This is a pumping unit wellhead. Pumping unit is used to lift the brine to surface. This well is an unauthorized annular disposal well. There is a 1000-gallon poly tank which drains brine into a 1-inch black, poly tube that is directly connected to the annular space between the 8-5/8 inch surface casing and the 4-1/2 inch production casing. Unauthorized annular disposal of brine is a violation of 1501:NONE(1)/ 1509.22(D). The operator shall immediately cease the unauthorized brine disposal. Physically disconnect this line. There is no identification sign for the well at the well site, either on the wellhead or the 1000-gallon poly tank. According to 1501:9-9-05(A)10/1509:23 the well identification sign shall include owner's name, county, township, well name and number, permit number for the well, and emergency contact number. Further, the liquid/gas separator is 5-feet from the pumping unit wellhead. According 1501:9-9-05(A)5/1509:23 mechanical separators shall be set at a minimum of 50-feet from the well. The operator shall move the separator to comply with this distance regulation. 9-9-05(A)10

1199916749 2011-03-01 Called Equity Oil & Gas at 11:33 am, and left message for Alane King to call me back regarding the well status. Production records have not been sent in since 2005. I left my phone number on the message.
1269i1032 2002-05-15 Follow-up on NOV's , Non-Compliance, well still idle and incapable of producing, according to landowner company has not been around for 2 to 3 months. All work has stopped on well, lease rd. is eroding into ST. RT. Road ditch and well site has not been restored to prevent erosion. No ID at the well or on tank, bottom of tank starting to leak.

1358453934 2013-01-17 Mr. Eugene Baisden called Joe Hoerst who then referred him to me. His complaint was that he had no domestic gas from this well for a month. He contacted John Murphy Equity O & G representative about the situation. Mr. Murphy had his well tender, Craig Ross, examine the wellhead to determine the problem. The electric motor needed to be replaced on the pumping unit. They obtained the motor but couldn't connect with Mr. Baisden during the month to install it. Mr. Baisden felt he was getting the run-a-round from Equity O & G on this issue. I asked Mr. Baisden if the well was leaking or there was some other problem related to the well. He indicated to me that the only problem was no gas from the well. I told him that this was not a problem covered under our responsibilities but I would call the operator's representative, Mr. Murphy and find out the progress of resolving this issue. Mr. Murphy informed me that Craig Ross was returning to Mr. Baisden's residence today to fix the problem and install the new electric motor. Mr. Murphy informed me that Mr. Ross had contacted him that he was coming over to this well to perform the necessary repair. I called Mr. Baisden back and explain to him that he needed to remain at his residence until Mr. Ross arrived to install the motor. I asked him to call me if this does not happen. I entered this complaint into the complaint log. I will follow up on this when I am in the area performing other inspections.
1359562463 2013-01-29 Mr. Eugene Baisden contacted me today and told me Equity Oil and Gas representatives installed the motor to his pumping unit, on 1/26/2013, and he is receiving gas to his house from the well. This initially was a phone complaint that I fielded. I contacted the well tender and his boss to get the issue resolved. This activity was recounted in Inspectors report #1358453934, 1/17/2013. No violation was submitted on RBDMS as this was not an issue for Oil and Gas Division. I did file a complaint on the Complaint Log when I received the complaint on 01/17/2013 and closed it today (1/29/2013).
1958895074 2013-05-29 Returned to site to confirm that Equity Oil & Gas representatives disconnected the annular disposal poly tubing from the 1000-gallon poly tank and the annular disposal connection to the wellhead. Craig Ross, Well Tender for Equity O & G informed Jon Scott and eventually me that they had performed this task on Tuesday after Jon Scott and I had collected water samples from the tank. The ball valve at the wellhead where the disposal occurred was shut-in and the poly hose was cut about a foot from this connection and then bent and stuck into the top opening of the tank. Took photos to compare with the original connections. Landowner informed me and Craig Ross confirmed that the old access road will be refurbished to allow movement of equipment to the wellhead without going through landowner's drive. GPS of access road off 243 when entry is ready is 38.49817/-82.51390 accuracy +/- 9.0 feet.
