11579088 2002-04-09 4-4-02 - received a spacing and odor complaint from Paula Dubaniewicz, 1846 N. Cleveland-Massillon Rd., P.O. Box 26, Bath, Ohio 44210. Phone # 330-666-0174. 4-9-02 - Inspectors Claus and Worstall met with Paula Dubaniewicz, James Paulett (Chief, Bath Fire Dept.), and George Seifert (Bath Fire Dept.), regarding odors from a tank battery located southwest of Paula Dubaniewicz's barn/art studio. The distance from the corner of the horse barn to the nearest stock tank was measured at approximately 92 feet. Crude oil odor was present in and around the barn/studio at the time of the inspection. The Firestone #6-F and #7-F wells are new production wells that flow into the multiple tank battery. There is NO VIOLATION in regard to the spacing requirement for the tank battery to the barn/studio. Steve Swain (Oil & Gas Solutions) will be contacted concerning the complaint and a suggestion will be made to place vent stacks on the 140 BBL. stock tanks to eliminate the odors.

2479041 2002-02-13 2-12-02 Received anonamous complaint (Case # 02-003) about drilling pits. 2-13-02 I inspected the location and found workers pumping fluid out of the working pit. The blow pit was closed. NO VIOLATIONS. I met with Steve Swain (Oil & Gas Solutions) and informed him of the complaint and my inspection. Steve said that they will be solidifying the working pit as soon as possible.
