-1025257641 2012-11-16 I inspected this well that had been plugged the previous day and found that small bubbles were coming from the cement surface at the top of the well. I tested the bubbles with a Honeywell PHD 2 multi gas detector. The readings were 0% LEL, 0% CH4 and 0% H2S. The bubbles may be from trapped air from the cement curing process that is now escaping. I advised Harold Moravy, consultant for Landmark 4 LLC, and advised him of the bubble situation. I will be monitoring this situation daily for the next few days.

-209622402 2013-08-02 There was no indication of the well, tank battery, or lease road. The grading and vegetation have been restored to natural conditions. The site is located on the south side of a sanitation facility for the city of brunswick. Final restoration passes.
-667992045 2012-10-24 I inspected this well and found one pump jack with electric motor on the wellhead. The electric power to the pump jack motor was disconnected as there was no electric line running from the nearest pole to the electric panel for the pump jack. I could not determine the pressure on the casing or tubing due to both gauges being broken. There was a chain link fence with a locked gate surrounding the well and an identification sign was posted on the fence. The area around the pump jack was well maintained, but there was a pile of used bent and broken steel gathering lines to the rear of the well. There also was an old drip tank with regulator and piping still attached laying in the nearby woods. No visible leaks were detected at the wellhead or piping. The electric lines for the pump jack were hanging down no higher than 10 ft. in the air across the access road and were attached to trees, not electric poles. The tank battery was gone and the area where the tank battery once stood was recently excavated and rough graded. The storage tanks and production equipment were removed and gone. I discovered numerous ripped up disconnected steel gathering lines sticking up from out of the ground in the vicinity of the former tank battery. There were several lengths of steel well tubing and casing laying near the excavated area. This former tank battery also handled production for the E P Fuller #2 well. The operator of the well, Landmark 4 LLC, has a current plug permit on file with an issue date of 09/21/2012. I attempted to contact Steve Swain of Landmark 4 LLC and left message with his office to have him contact me. I received no return call.

1354294564 2012-11-13 I inspected this well today and found that Thompson Well Service had moved the service rig onto the location and rigged up for the well plugging operation. The well location and the lease road were extremely muddy. After rigging up, the service rig crew pulled the production rods from the well and laid them on a trailer. The service rig crew then hooked onto the 2 in. tubing and removed one joint in preparation for placement of the bottom cement plug. The 2 in. tubing was set at 3236 ft.
1354552129 2012-11-14 I inspected the well plugging operation and found that the Landmark 4 LLC crew was moving a 150 bbl. water tank onto the location. The site was extremely muddy and moving any equipment around on the location was a difficult task. Landmark 4 LLC pumped 100 bbls. of fresh water into the tank for the plugging operation. The crew then began construction of a 10 ft. x 8 ft. x 6 ft. pit and lined it with a 12 mil. plastic liner. Petroset Cementing arrived on location and set up their equipment and hooked onto the 2 in. tubing. The Petroset operator was instructed by myself and Harold Moravy, the consultant for Landmark 4 LLC, to pump fresh water and break circulation. Apparently, there was a miscommunication because after pumping the water ahead of the cement the Petroset operator went ahead and began pumping the cement before circulation was established. I did notice that gas vapors were coming to the surface around the casing before the cement was pumped. Petroset pumped 25 sks. of Class A cement with 2% gel and placed the plug from 3236 ft. to 3036 ft. The cement was displaced with 11.5 bbls. of fresh water. The Thompson Well Service crew then tripped out the 2 in. tubing from the well and stacked it on a trailer. A total of 105 joints of 2 in. tubing plus one mud anchor joint was removed from the well. The service rig crew then hooked a 5.5 in. ripper tool and ripped the 5.5 in. at a depth of 2120 ft. The service rig crew pulled 30 ft. of 5.5 in. casing and then hooked up Petroset Cementing. Petroset began pumping fresh water and broke circulation and flushed 1 bbl. of crude oil into the pit. Petroset then pumped 80 sks. of Class A cement with 2% gel. The cement plug was placed from 2100 ft. to 1800 ft. and was displaced with 43 bbls. of fresh water. The service rig crew then pulled approx. 600 ft. of 5.5 in. casing from the well and stacked it on a trailer.
1360239692 2012-11-19 I inspected this well that was plugged on 11/15/12 and found that the air bubbles that were found coming to surface from the cement on 11/16/12 were now smaller and much less frequent. The bubbles were coming at approx. 5 minutes apart. The bubbles were tested with a Honeywell PHD 2 multi gas detector. The readings were still 0% LEL, 0% CH4 and 0% H2S. I will check on the situation again in several days.

1977532765 2012-11-26 I inspected this well that was plugged on 11/15/12 and was found to have bubbles coming up through the cement on 11/16/12. I now detect no bubbles in the small amount of water that had accumulated on top of the surface cement plug. I advised Harold Moravy, consultant for Landmark 4 LLC, of the situation and advised him to have crew proceed to remove the top 36 in. of the plugged casing to complete the plug job.

574457966 2012-11-15 I inspected the plugging operation and found the location and lease road to be extremely wet and muddy. Petroset Cementing arrived on location and had to be towed in by a bulldozer and pushed by a trackhoe because of the muddy conditions. The Thompson Well Service crew pulled the 5.5 in. casing up to a depth of 550 ft. Petroset hooked up their cementing equipment and pumped fresh water and broke circulation. Petroset then pumped 55 sks. of Class A cement with 2% gel and placed the cement plug from 550 ft. to 350 ft. The plug was displaced with 8.5 bbls. of fresh water. The service rig crew then pulled approx. 550 ft. of the 5.5 in. casing and stacked it on a trailer. A total of 63 joints of 5.5 in. casing was removed from the well. The service rig crew then hooked up an 8 in. ripper tool and ripped the 8 in. casing at 298 ft. The crew pulled 16 joints of 8 in. casing from the well and stacked it on a trailer. The service rig crew then ran 1.5 in. tubing to a depth of 298 ft. in order to place the cement. Petroset hooked up their cementing equipment and pumped 175 sks. of Class A cement with 2% gel from 298 ft. to the surface. I did witness the cement return to the surface. The service rig crew removed the 1.5 in. tubing and stacked it on the ground. After approx. one hour the cement was still holding at the surface.
