-698820248 | 2009-08-18 | On 8/16/09 in the late afternoon I received a call from LT. Jim McCreary from the Boardman Fire Dept. pertaining to a line blow out on 8/16/09. On 8/17/09 I inspected the site and talked with George Strawn, Everflow Eastern. He stated that the logic(Computer to run the well) on the wellhead had failed. The well cycles every 45 minutes. The production line built up pressure. Then separator pop off failed. The 2 inch line off of the separator parted and blew gas. No brine spill or fire. Gas only. The Boardman fire department was on the scene. George Strawn was called out. The repair of the tank battery started Monday, 8/17/09. A different separator and pop off were installed. This complaint is resolved.

1951059769 | 2012-02-06 | I conducted a status check on this site and found that the well head was fenced with screened chain link, and it is equipped with a plunger lift system. The tank battery consists of one 50 barrel stock tank equipped with flash arrestor and charcoal filter, one 50 barrel poly brine storage tank, and one separator. The thief hatch was closed and there was a bull plug installed in the draw valve. The tank battery was fenced with screened chain link, and the dike was free of vegetation and debris. Proper identification was posted, and I found no leaks or spills at the well head or the tank battery. I took new GPS coordinates of the well head and tank battery. No violations at this time.
