-267798062 | 2013-10-15 | I inspected this well to follow up on Chief's Order #2013-136 issued on 9/25/2013. The operator, Stephen J. Svetlak, LLC, has failed to maintain proof of liability insurance as required by ORC 1509.07(A) and has been found in material and substantial violation for failing to maintain coverage. No commercial production has been reported for this well since 1997. My inspection found a tank battery with 2-100 barrel stock tanks completely overgrown with brush and trees. The overgrowth was such that I was unable to get within 20 feet of the battery. After searching the brush to the north and east I was able to locate a horizontal separator that was in disrepair and not being utilized. I located a Dominion/East Ohio line marker and an old domestic meter in the brush. The meter did not appear to be in use. After further searching, I located 4.5" casing above grade and equipped with a 4"x2" swedge and a 2" ball valve. The ball valve had a small leak and was hissing. The well head had been removed and pieces of it were lying on the ground. The disposition of the well and equipment is safety hazard due to the overgrowth and the leaking gas. This well is idle and needs to be placed back in production or properly plugged and abandoned. |
