1361807091 2013-02-23 My inspection of the injection facility and brine haulers was accompanied by Stephen Tompkins. While we were on location, five brine trucks unloaded. McKissick Trucking, Shortfuse Gasfield Trucking, and Myers Well Service, each had one truck unload. There were two trucks owned by Bessemer Supply. All trucks were registered and correctly identified. All conditions at the facility were acceptable.

323964061 2013-05-14 I completed a salt water injection well inspection with ODNR Inspector Steve Ochs. My inspection found the tubing had 750 psi and the 4.5" had 170 psi. The well was surrounded by a locked board-on-board fence with the proper identification sign. The injection facility was in good operating condition with no water or oil surrounding the diked tanks.
