-156765122 2013-04-03 My inspection found the well in production. It was equipped with a plunger lift unit and surrounded by a chain link fence. The tank battery contained one, 100 barrel production tank. It was surrounded by a chain link fence and a dike capable of meeting containment requirements. The drain going through the dike had a broken threaded cap. The heat, treated separator had the correct spacing and contained a 7 day production chart. It was surrounded by a chain link fence. All three fences needed new locks. I called Dick Helderman at 330-807-4224 and told him to install new locks at the fences. The lease road was in stable condition. This well was placed into production prior to House Bill 278.
