480759108 2012-10-16 My visit to the well today was twofold. The city of North Royalton had requested that Bass Energy relocate the access road to this well due to construction on city property adjacent to the original access road. Bass Energy notified me that this was accomplished in late August of 2012. The access road was constructed to best management practices for site construction and meets requirements of 1509:09-9-9-05. I conducted a well inspection while on location. Well head location is clean and in good operating condition. There is 180 Pounds pressure at the well head to the feed line. The tank battery has two (2) 140 Barrel storage tanks and caps are secure and in good condition. There is one (1) fifty barrel brine tank. There is a separator unit and a gas feed line unit. Fencing is chain link eight feet tall with three strands of barbed wire surrounding the top. Well is in compliance. No violations found.
