-521998742 2006-06-07 Dike constructed around the 2 tanks does not appear to have sufficient capacity for spill storage. The dike walls are dirt with little vegetation. Another tank has been set up in the area and is located in an excavated pit that appears to be adequate capacity for the tank. A permit sign is in place at the tanks but has incomplete information. The laminate on the sign is separating and the sign will deteriorate quickly. Recommend contacting inspector at 419-599-0058 to discuss situation.
1118331052 2005-06-09 Earthen dike is being constructed around the tanks. Spoke to Mr. Mauch. Dike will be graded and seeded to prevent erosion. Bond is in the process of being submitted. Reminded Mr Mauch that ID signs need to be posted at each well. He indicated that one of the wells on the property has not been produced since he bought the property.
590772412167862 2005-04-06 This is an unpermitted well located on the property of and operated by Mauch Farms Inc. The well is being produced and will be desiginated as well # 5. There are also 2 storage tanks near this well. A dike will be required to be constructed around the tank battery to contain any spill. The dike should have the capacity to contain the amount of liquid for the largest single tank or if the tanks are connected, the amount of liquid of the combined tanks. The operator will submit the necessary information to obtain a permit.
