Contacted Bruce Levengood and advised that the site will need further grading and seeding before the final restoration can be passed.

I did not find any leaks but I did find that the 8 5/8" inch diameter surface casing needs to be cut off 36" inches below ground level and the remaining equipment removed. Contact Sound Energy and advise them that the casing can be cut off.

The well operation has pollution inside and outside the well head dike. The tank battery was not located. According to land owner, Ken Fouts, the tank battery has been removed. 1344948962 2012-08-13 I am on site to respond to a call from OEPA that was called in this afternoon. I found that there is a 28 ft. x 12 ft x 2 ft dike around a non producing well. The dike has 8 to 15 gallons of crude oil and brine inside the contained area. I contacted Bruce Levengood @ 330-364-7665 and advised of the on going contamination at his well location. He said he would call for a water truck and other equipment to remove the oil and contaminated soil in the dike and around the well. He said he felt he could be on site Tueday 08/14/2012 or Weds at the latest. He is to contact me when he is going to be conducting the clean up activity so I can be on site.
1367418677 2012-11-20 I am on site today to witness the plugging of this well. Berry Well Service has a service rig on location and Omegea Cementing is on site to pump cement. The well has 2 3/8" inch diameter tubing in it. The tubing has parted and could not be fished out. So it was determined that the 4 1/2" inch casing would be loaded with freshwater and if it went on vacuum a plug could be sent to the bottom even though tubing is in the hole. A string of 2 3/8" inch tubing was installed to 1469 ft. Omega mixed 100 sks of Class A cement and displaced it to that depth. The well was packed off and the 4 1/2" inch diameter casing was pumped up and Omega displaced 54 bbl of water to place the cement to the perfs. Then the tubing was pulled and the casing ripped at 1469 ft. tubing was run back in the well and a plug was set from 1163 ft to 1463 ft. The last plug was from 200 ft to surface. Cement was circulated to surface where it remained after the job was complete.
