-1581819304 2009-12-09 On site for long string job. Cementers were completing job when I arrived on location. Checked drainage controls on site. Lease road is extremely soft there is approximately 1 to 3 feet in depth of mud and water on top of the lease road from the end of the rock portion of the road to the site. There is 2 to 6 inches of mud on top of the rock portion of the lease road. Mud is flowing onto the landowners lawn, south side of the lease road. I instructed Jeff to put up silt fence. Operator is going to put up silt fence in a horse shoe, cut the edge of the lease road so that drainage will flow onto the lawn and be controlled by the silt fence. Last 75' of the lease road slopes down to the road, drainage was flowing to the end of the lease road and ponding. Silt fence sump on the lawn will control drainage. Spoke to Mark from Petroset, long string job went well no lose of circulation. 200 Sacks of class A cement was ran.
-176545272 2010-05-26 On site in regards to an odor complaint in the area. Checked Tank battery LEL and H2S zero. No odor noted. Identification sign is posted on the 100BBl tank in the battery. Lease road to the tank battery is stable. Permit 22110 is also produced into the battery. There is a PVC drain pipe in the battery with no caps or valve. NOV has been issued for permit 22110, the dike is not sufficient to prevent to the escape of brine or oil.
-35751184 2011-06-09 Revisit site of oil and brine spill at wellhead due to gauge blowing off. Tank battery contains 2 100 BBL production tanks, Rickard #1 and Rickard #2, and mechanical seperator. Dike is good, feed lines are bullplugged. Spacing is good. 1 absorbent diaper in place at base of wellhead.
-828159564 2011-06-01 Well head has a leak at a fitting to a control box. 1/2 inch line was leaking were it threads into the box. Lease road needs final grading and seeding.
107344249 2009-12-10 Ravenna road was clear of mud. Operator was on site finishing drainage controls. Silt fence was up, operator was digging shallow swales to direct drainage into control area. Weather change temperature was in the low 20' drainage was frozen, lease road was starting to freeze up. Operator has graded lease road. Operator is going to shut down until Monday and wait for site to freeze before moving to 2nd location. Operator is going to put silt fence up around 2nd site, down slope side to prevent offsite erosion.
1202395452 2009-12-02 On site with supervisor Jay Cheslock. Walked property north of the lease road to check drainage. Lease road is causing ponding of water on the south side of the road in the woods. Drainage is contained in the woods, no flow of drainage was noted. Vernal pools were noted in the woods. Called company at (440) 667-3381 Left message requesting an additional 100' to 200' of lease road be added. Lease road is gravel, the portion that is a private drive, drive is approximately 100' long. Operator has added additional stone to the private drive since setting conductor. At the end of the private drive lease road is clay and is approximately 500' long. Lease road is cutting 1' to 2.5' into the clay soil. Lease road is extremely soft. Operator has been attempting to back blade the lease road. Additional rock lease road needs to be constructed to prevent tracking mud onto Ravenna road. Was able to follow mud trail for approximately 1/2 mile on Clark Road to the site on Ravenna road. Operator has scrapped the majority of the mud off of the road. I fine coat of dried mud was from Clark road to Ravenna road. I parked on the gravel portion of the lease road and still tracked mud onto the road when I left the site. A 2nd well is going to be drilled on the property, lease road needs to be stabilized.
1260471709 2009-12-08 Rig drill depth 3670'. Pits are in good repair. Drainage is staying on site. Lease road is extremely soft, lease road is cut 2.5' to 3.0' feet into the ground. Site is muddy and soft but in good order. Operator twisted off earlier in the week, driller was able to recover drill string and continue drilling.
1804682046 2013-06-21 My inspection found the well to be equipped with a plunger lift unit. The well and tank battery are in good repair. The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. The tank battery and lease road is being shared by permit number 2-2110. The location has been rough graded and vegetation has been established. Preliminary restoration is passed.
799296154 2009-11-30 On site for CND job. Job went well, 1 to 1.5 BBL of cement returned to surface, cement held at surface. Pit liners were in good repair. Questioned Jeff from Autumn ridge regarding water in the pits. Pit water had a red and black scum floating on top of it. Questioned Jeff if water was fresh or brine. Pulled water sample, water was not salty, fresh with gel. Scum was rust and residual from the water trucks and old fresh water tanks. Instructed Jeff to skim scum from the fresh water and put it into the back pit. Reminded him that only fresh water can be used to drill CND and Surface holes, fresh water with scum from tanks is not acceptable, all precautions must be taken to prevent contamination of the fresh water.
