-824013536 2007-12-04 -Cancelled N.O.V. # 1080591111 issued 4/01/04 for well not in production & not labeled. Operator ordered to either produce or plug & ID the well. Temple Oil contracted by well owner Power Gas Marketing-North Coast Energy to plug the well. 64' of 10.75" surface casing and 302' of 7" casing left in hole but for upper 30". No production casing in hole. Cemenco Services, Inc. cemented the well. Tremie pipe used to pump cement for the plugs. Class A cement w/ 4% bentonite gel additive used. First plug was from 1300' to 1066' w/ 35 sacks of cement. The second plug was from 402' to 202' w/ 30 sacks of cement. The third plug from 100' to 30" below the surface used 25 sacks of cement. Freshwater used as spacer w/ 3 bbls between bottom & middle plug & 0.75 bbls between middle & upper plug. Bridge plug used beneath the upper plug. Pit dug for excess fluid but no fluid circulated. 300' of water bailed from the well. Well to be cut off to 30", ID cap added, hole & pit to be filled, graded, seeded & mulched. Final restoration insp to be on 12/7/07.
1080591111 2004-03-22 -On site for status check on the well with John Firko of Ohio Power Gas (aquirred EDC in the mid-1990's). Also met with Bob Huwer of Huwer Drilling. Well last reported gas production in 1991. Valves shut off. Some gas pressure when valve opened. Regulator broken off. 1.5" Roots meter broken off. Drip valve (7" to 2" valve) 600 lbs brass Apollo valve present. 8" drip valve. ID tag washed out. Digital photos taken of the well. Well currently incapable of production. Operator to investigate if he wants to repair & put well back into production, if there is a market for the gas. If not well to be plugged. Well permit approved 8/16/90. Well spudded 8/28/90 and TD reached 8/29/90. Well completion report dated 9/24/90. TD to 1300 as reported by drillers & loggers. 1197552191 2007-12-12 -Well plugged on 12/4/07 by Temple Oil & Gas with Cemenco Services. When leaving site on 12/4, operator was filling in the pit. Well later cut off at 30 inches below the surface, and cap placed. Well location filled back in and pit filled in and graded since well plugged on 12/4. Access lane to well graded out. Production equipment removed. Well was in a plowed field so no reseeding required. It will be planted in the spring. Photos taken. Will recommend approval of final restoration. Sign affidavit for Dana Blue with Temple Oil & Gas to submit to well owner.
