1341923298 2012-07-10 On 7/10/2012, I conducted a production well inspection status check. This well shares a tank battery and lease road with permit number 34-055-2-0286-00-00. My inspection found the pumping unit has been removed and the well was shut in. The tank battery contained one 210 barrel oil storage tank with no identification posted, one 30 barrel brine tank, and two separators. I called Bocor, at 330-494-1221, and I was told the well is producing and has become a swab well. I asked for the well tender to install the proper identification. I will perform a follow-up inspection by 7/20/2012.
1381409692 2013-10-09 I responded to an inspection request from the landowner for a possible oil spill at the tank battery. My inspection found crude oil around the two separators and next to the 30 barrel brine tank. The spill was completely contained inside the dike. According to the landowner; he claimed the valve on the load line at the brine tank was open, which he closed after discovering the spill. The spill appears to have originated at the brine tank. I found the 2" plug for the load line valve was lying on top of the brine tank and a 4" diameter depression on the ground from the oil spilling out. I immediately called the well tender, Jim Palmier, at 330-705-3011 and Todd Sutko, Bocor field supervisor, at 330-506-2531. I instructed both gentlemen to start clean-up operation immediately. Todd stated Bocor would start mobilizing equipment and begin cleaning up the oil spill first thing the next day (10/10/13). I took photos of the oil spill.

302740170 2013-10-11 My inspection found all of the impacted soil piled up at the south end of the tank battery and ready to be removed once the dump truck arrives. Both of the separators were disconnected and moved to remove any impacted soil. Bocor had peat sorb spread through the affected area. Todd Sutko stated new gravel would be spread.
