985695284 2013-02-13 My inspection found the well equipped with a plunger lift unit. The tank battery contained one, 100 barrel production tank. The dike was weathered and not capable of meeting containment requirements. Also, it was overgrown with weeds and small trees. The heat treated separator had the correct spacing and contained a 7 day production chart. The lease road was in stable condition and shared with permit number 2-1306. The production records were up to date on our database. This well was placed into production prior to House Bill 278. I called Bob Giering at 330-503-4340. I instructed him to have a need dike build by 2/22/2013. Also, I told him to have the weed and small trees removed.
