-1303283728 2012-06-11 My visit to the well was to inspect well site construction and rig move on and set up. Best Management Practices for site construction are being employed, in addition pre permit conditions are being adhered to. Rig is on site and is beginning to rig up.
-312632495 2012-06-13 My visit to the well was to witness the cementing of the surface casing. Cement reports will be filed upon completion of the well drilling process. I also witnessed the Blow Off Preventer test on the evening of 06/13/2012.
-54215457 2014-07-07 I inpected this producing unit as part of routine inspection activity and also as a follow-up to my last inspection. At the time of my last inspection, production of this unit had been stopped temporarily pending resolution of a technical problem with the metering connection with Columbia Gas. The well is currently still idle pending the solution. I did not observe any problems, leaks, spills, or other issues.

1371998397 2012-06-13 My inspection found the rig shut down and waiting for cement to set. They will conduct the Blow Out Preventor test later tonight (tentative time of 8 PM). DNR Inspector Ray Wallce is schduled to witness this test. No violations found.
1378400337 2013-09-05 The producing unit consists of a producing wellhead with pumpjack and aboveground storage tank battery. The wellhead and tank battery are enclosed in separate chain-link fences. The battery is comprised of three 100-barrel production tanks and one 50-barrel brine tank. The secondary containment is good. 1501:9-9-05E safety equipment is present. There is no screening at the wellhead or battery. This will be required if not already waived. Preliminary restoration is pending verification.
1385402919 2013-11-25 This unit reportedly has been shut-in following a valve failure at the sales line which caused oil to infiltrate Columbia Gas distribution network. M & M has requested an allowance to operate a flare on a temporary basis which would allow them to continue to produce oil from the unit.
