-113193326 2014-02-06 I was on location on 2/6/2014 to witness the plugging of this well. Nick's Well Plugging was plugging contractor on location. Nick's rigged up and bullheaded the bottom plug with 35 sacks of Class A cement and 50lbs of cottonseed hulls. The plug landed and the well pressured up to 1500psi and was shut-in for two hours. Nick's service rig ripped the 4.5" casing at 2668' and pulled 5 joints to 2462'. Nick's rigged up, broke circulation, and pumped 86 sacks of Class A cement to cover the top of the "Big Lime". Nick's pulled 45 joints of 4.5" casing, broke circulation, and pumped 60 sacks of Class A cement to cover the bottom of the surface casing. Nick's pulled 10 joints of 4.5" casing, broke circulation, and pumped 62 sacks of Class A cement, cement circulated to surface. The remaining 5 joints of 4.5" casing was pulled from the well and the well was topped off with cement. A total of 65 joints of 4.5", 10.5lb casing was recovered during the job. GPS coordinates are +/- 8'.
-148979044 2014-07-15 I was on location to check on the status of this plugged well. The tank battery still remains intact. The area where the well was located needs to be graded and seeded. The tank battery needs removed. I contacted Mr. McDaniels a supervisor for Enervest about the final restoration.
-374342125 2013-05-20 My inspection found the well to have the 4.5" plumbed into a swedge. The main valve was shut and the well did not meet minimum production for 2012. The tank battery is surrounded by a sufficient dike and a chain link fence. The tank battery consists of a 210 BBL production tank and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. I sent an email to Mr McDaniel from Enervest to find out what the plans are for the well.
1303706726 2014-02-12 My inspection today found that Nick's Well Plugging has completed the plug job on this well. The area has been graded. Once the weather changes Enervest can complete the final restoration process. No violations found.
1378737320 2013-09-09 My inspection found the well to have the 4.5" plumbed into a swedge. The main valve was shut and the well has not shown production since 2005. The tank battery is surrounded by a sufficient dike and a chain link fence. The tank battery consists of a 210 BBL production tank and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. The well must be produced within 30 days or plugged within 90 days of this notice. NONE
1474250400 2012-09-27 The unit consists of a 1-tank battery (TB) with mechanical separator located in a 6-foot chainlink fence. The spacing is in accordance with the rule. The unit ID indicates the owner is Great Lakes LLC. I could not locate the wellhead. I did not observe any signs of leaking product or gas vapors. The tank battery dike is adequate. I contacted Tom Havraenk at Enervest regarding maintenance of ID.
38382012 2014-01-27 This inspection is a follow up to an inspection that a compliance notice was issued on 9/9/13. My inspection found the well to have the 4.5" plumbed into a swedge. The main valve was shut and the well has not shown production since 2005. The tank battery is surrounded by a sufficient dike and a chain link fence. The tank battery consists of a 210 BBL production tank and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. The company has been issued a plugging permit for the well.
