-479404949 2014-05-28 Upon inspection, it was observed that there is now legible and correct identification information posted at the tank battery for this well, which is approximately 75' from the well head. The "R" style well head setup is currently disconnected from the 1.5" tubing and is allowing the 2" fitting from the annulus to vent gas into the atmosphere. In addition, a small amount of oil and paraffin has leaked from this fitting onto the ground surrounding the well head. I will contact Altheir's Oil Inc. and notify them of the situation.
1396955695 2013-04-22 This is a pumping unit wellhead. There was no identification sign at the wellhead. According to 1501:9-9-05(A)10/1509:23 the well identification sign shall include owner's name, county, township, well name and number, permit number for the well, and emergency contact number. There is a t-fitting at the surface. There are two 2-1/4 inch pipe coming from the side connections of this fitting. A 1-7/8 inch production tubing rises above the cap of this fitting to another t-fitting. Out one side is a 2-1/4 inch production pipe. On the other side is a 1-1/8 inch tubing ending in a ball valve and plug. Attached on top of this fitting is the stuffing box assembly for the pumping unit. At present the wellhead has no motor. Tank battery next to the wellhead consists of one 100-barrel tank , one liquid/gas separator and a small knockout tank in the containment area. There is also no identification on anything in the tank battery. 9-9-05(A)10
217716977 2004-11-05 Well is turned on; two IDs on tank for Altier - one is for this well. Gas to motor off but landowner has seen pump jack operating. Need to change status and ownership for this well.
