-1748892169 2014-06-19 On the date of inspection I accompanied ODNR Inspector Mike Hartshorn to witness a surface casing string. When we arrived on site we were informed that the casing crew had damaged tools and would be unable to perform the cement string until later. Inspector Hartshorn and I left the site and would have to return when the casing was set correctly. Refer to Mike Hartshorn's report for further details and witness.
995875714 2014-05-09 On the day of my inspection, I witnessed Universal Well Services break circulation with fresh water then displace six hundred and three sacks of Type 1 cement to a depth of 708 feet, I witnessed 22.0 sacks of cement returns to surface, site inspection revealed no issues.
