-941351518 2013-01-30 My inspection found the well equipped with a plunger lift unit. The tank battery contained one, 100 barrel production tank. It was surrounded by a very weathered dike not capable of handling containment requirements. There was a heat treated separator with the correct spacing from the production tank. The lease road was in stable condition with a locked gate at the entrance. The production records were up to date in our database. The well was placed into production prior to House Bill 278. I called Doug Trevis of RMI Titanium Company at 330-544-1165. I gave him until 2/13/2013 to install at new dike.
