-1220161767 2013-05-07 I talked with Joe Melville and he stated that they will either patch the casing, plug the well, or give the well to PDC. Either way, they will have the issue resolved in a month.

Power lines are still up and there are a few production lines still laying on the ground. The access road has some grassy areas but still needs a lot of work. I talked with Joe Melville and he will contact Donny with Knox Energy.

At the access road the separator and meter box are disconnected. The well was shared with #4338 and there are three 100 barrel tanks and one 1500 gallon tank in the dike. Operator must post ID and make well capable of production within 60 days; or plugging is required in 60 days. I talked with John Melville, the field manager, and he said they plan on plugging the well and will get the plugging permit to the state in the next 30 days. NONE

1380032061 2013-09-23 Knox Energy single pole rig was plugging well. They had 4.5casing ripped off and ran tubing of 2.375upset to 2591'. We set a Lime plug at 2591-2291'. The Berea plug was set at 1478'-1290'. The Bottom of the surface plug was set at 673'-473'. The surface plug was at 200-0. Formation ran total of 125 sacks of class A cement at 2%gel and 125 sacks of cement at 1%gel cement. All plugs circulated.
2056977507 2013-07-11 Compliance Action Follow-up -301183717: There have been no resolution to any of the issues. 7/12/2013: I talked with Joe Melville and he stated he will be coming down next week. PDC did not want this well so they are going to file for a permit to plug. He already has Knox Energy lined up with a plan to plug the well sometime in August. 8/28/2013: I have received the permit to plug this well.
239762463 2013-09-19 I arrived on location with Cemenco to witness the bottom hole plug job. The bottom of the tubing was set at 4795' and a 400' cement plug was set from 4395-4795'. There was good circulation with oil coming up the annulus most likely from a hole somewhere in the 4.5". Knox Energy will pull the rest of the tubing from the hole then come back tomorrow to try and rip pipe starting somewhere around 3500'.
