1275620993 2010-06-03 On day of inspection found sheen in creek along county rd.45 from Rich Fork north to tank farm. I was not notified. There were oil soaked diapers still laying in creek. Took pictures of site. Found 4-100 bbl tanks and the tank that had been ran over. There is no dike present. The power pole is directly above the tanks and is washing out. This could be a possible tank fire. Tanks need to be moved and diked up. Remove oily diapers from creek and contaminated soil as soon as possible. Dike needs to be 110 %the capacity of 4-100bbl tanks. Notify me as soon as possible for dike measurement. Dan Goins and I were the inspectors. 9-1-07

1283337539 2010-08-25 On day of inspection well was supplied with a pump jack,6 1/4 cas,2tubing and rods. There are two 100bbl tanks that were diked up and moved away from creek.NOV has been resolved. Old tanks were removed.
