Mr. Moran agreed to resolve, as this well had exhibited the same conditions back in February. (Note: While pumping since February, pressure would be observed on the injection side and Mr. Moran would add fluid to annular space resulting in no vacuum.) -1888934474 2009-09-30 Jeff Fry and I inspected the well location at the request of Tom Tomastik. We met with the landowner and well owner Phil Moran. He explained his intentions about converting his Clinton well into a salt water injection well. We discussed the area to be used for the unloading pad, storage tanks, dike construction, and the plug back procedures for this operation. Also, we took GPS coordinates for the nearest water well, which is on Phil Morans property.

-2130956941 2012-02-06 An inspection revealed that well contained vacuum on both injection and annulus sides of well. Note: Pump not running at time of inspection. The annulus valve was opened for approx 10 seconds with constant vacuum. After checking the injection side of well and observing vacuum, the annulus valve was again opened and verified presence of vacuum. At 9:37 am, I left Mr. Phil Moran a voicemail message. Mr. Moran would check well later this evening while pump was running and observed vacuum on annulus. However, states it eventually blew down. Further stating 200 psi on injection and filled fill annulus with approx 15 gal of water. Agreed to meet at well location tomorrow for further review.

A concrete unloading pad has been constructed with a curb around the outside. There is a drain the pad that leads to the an underground tank. The lead line to the well has been placed on top of the ground but has not yet been pressure tested.

1256225688 2009-10-21 I witnessed the bottom hole plug today. There was 25 sacks of cement used to plug the Clinton by the bulhead method for 300 feet of fill up. The well is being plugged back to be used as an underground injection well. The pressure test is set up for 10-23-09. I also witnessed the construction of the dike area and the installation of the 30 mil liner and sand. Construction will continue tomorrow and into next week.
1256742774 2009-10-22 Phil Moran is completing the construction of the dike. Four inches of sand was placed underneath and on top of a 30 mil liner. Around the dike walls an additional liner was used. This second liner was placed on top of the sand and the 30 mil liner. Gravel screenings was then placed on top of this second liner. The dike measures 40 feet by 60 feet with approximately 40 inch side walls. It will contain three 300 barrel tanks and the pump.
1260808461 2009-12-14 I inspected the tank battery / pump house location of this injection well today. Phil Moran was not on location. The line between the tanks and the well need to be pressure tested. The pump was not running and there was zero pressure on the line. 882 barrels have been pumped. There were no violations noted this date.

1381313979 2009-10-27 Cemenco used there pump truck to pressure up on this well today in order to get the formation to brake and open up. The well was taking approximately 2 barrels a minute at 750 psi. Phil Moran, the well owner would like to see it do better than that and will treat the well with more acid. Producers came this afternoon and treated the well with 1200 gallons of acid and also pumped pressure on the well. The well then took about 6 barrels a minute with 750 psi.

1402315543 2014-05-29 Stopped at injection well and assisted Inspector Benko, and operator Phil Moran to pressure test injection line from pump house to both injection wells. This permit and permit 4603. While I was at the pump house a red semi tanker belonging to Dow Cameron arrived to unload. UIC 343, driver D. Grubbs. Truck was a red Peterbuilt license PHJ382, with a red trailer license TQA7107. At 10:15am I checked his hauler log. This was his second trip this date from frac tanks at their yard, storage Permit 2014-07 . Both times he had a load of 130 Bbl.. Log was correct but truck failed to have the Dow Cameron's phone number displayed per 1509.223. I informed the driver of violation. He then called their shop and had me talk to John Niceswanger. I informed John of requirements of 1509.223 and he informed me they would correct with decals and notify me when done. NONE..

1490905748 2009-10-23 A pressure test on the bottom hole plug was performed today. A solvent truck was used to pressure up on the well. At 9:25 am the test began with 715 psi on the well. At 9:40 am there was 705 psi on the well. The well passed the pressure test as there was less than 5% loss during the 15 minute test.

1942572568 2009-12-30 Jeff Fry and I inspected this well location today and performed a pressure test on the pipeline between the tank battery and the injection well. The pressure test passed. The test began with 620 psi at 9:32am and ended at 9:47 with 620 psi. Phil Moran was also on location. Phil still needs to post proper identification and the construction of this injection well facility will be complete.
2035819234 2009-10-26 Jet Wireline Services, Inc. perforated this well today. The well was loaded with acid and then salt water before being perforated. There were a total of 41 shots. They were 2 and 1/8 bi-wire shots. The perforation was from 2365 to 2405.

423129706 2012-04-12 I witnessed the construction and testing activity performed on this saltwater injection well today. Cemenco pumped packer inhibitor through 2 7/8" tubing and circulated to the surface. The well was shut-in and 625 psi was placed on the annulus. The pressure was monitored for the required 15 minutes with an allowance of 5% decline. After 15 minutes, my gauge showed 622 psi with only 3 psi loss. The well passed the required test.

775379107 2012-04-11 An inspection revealed service rig on location to pull 2 3/8" tubing and packer from wellbore, due to pressure equalization. During the retrieval, the top of packer came loose and fell down the hole. Mr. Phil Moran ran his tools in the hole and verified packer was on bottom of well. Witnessed the installation of used 2 7/8" tubing with new Parmaco Packer into the wellbore with Jeff Fry and Brandon Mitchell. Packer inhibitor and pressure test will be conducted tomorrow.
