-1048891859 2010-12-16 Quarterly status check inspection found no violations at this time. Injection pressure at the wellhead (970 psi) exceeded the allowable (850 psi) and the company shut the injection pump down. The pressure gauge at the injection pump indicated a pressure closer to 900 psi. The well reportedly is on vacuum at the start of the day and quickly builds to the maximum allowable injection pressure. The pressures need to be checked more often.

1093672158 2011-05-17 Quarterly status check inspection found that the permitted maximum injection pressure (850 psi) had been exceeded and the well was injecting at 1080 psi. I ordered the well shut in and the injection pump shut down. I also advised Ray Pander that if the higher injection pressures are due to injecting fresher water, he may consider applying for a fluid density modification to permit a higher injection pressure. I also advised him that additional reporting and monitoring requirements will apply if he chooses to pursue this modification.
1111441938 2004-05-04 Status check inspection of this salt water disposal well. No violations at this time.
1152534634 2006-07-08 Pander changed out the tubing string and replaced the packer in this well. The new tubing string is 2-3/8" upset and the packer is a rebuilt Arrow Lockset. Mixed five gallons of inhibitor in 80 bbl of fresh water and circulated to surface. The new packer was set in compression at 3738'. After setting the packer, pressured up the annulus to 1050 psi and held for 15 minutes. The test passed.
1152798167 2006-07-10 The injection system has been re-connected and started. So far injection pressures have been high. I reminded the driver on site that the maximum injection pressure is 850 psi.
1154391870 2006-07-25 The MIT was successfully completed on 7/22 and the well is now back in use. The injection pressures are near 800 psi and the annular pressure is 150 psi.
1216i108 2001-11-28 Arrived on site as the well was shut down. Pressures on the tubing and annulus are equal. Discussed with the well tender. Waited twenty minutes then turned the pump back on. Pressres went from 760psi to 820psi on both. Told the tender to shut down and determine the cause before injecting again. The tender called in to the office once to notify Ray or Allen, then again to vedrify that they were contacted. THE COMPUTER PROGRAM WILL NOT PERMIT THE ADDITION OF THE CODES VIOLATED. 1509.03 AND OAC 1501:9-3-05 ?.

1231266086 2014-05-21 The well owner reported an injection line leak. An inspection found a brine impacted area approximately 50 feet by 100 feet. A trench was cut down slope from the injection line and wellhead. The area was flushed with fresh water and the run off was collected in the trench, which was lined with plastic. 9-3-04(A)
1234072880 2003-06-17 UIC status check inspection - no violations at this time.
1243433047 2009-05-26 J-Well's service rig pulled the tubing and packer. I met with R.C. Pander on the site and discussed the well. The injection well is shut down until it can be treated on June 4th or 5th. I called Bob Worstall and informed him of the schedule change.
1247599445 2009-07-08 Pander removed the tubing string and packer from this well in order to treat the well for bacteria, crude oil and parafin. The tubing was out of the hole for approximately seven weeks while various treatment proposals were evaluated. The well was ultimately treated with bacteriacide and "Salty Dog" in an attempt to lower injection pressures. On 7/8 Hagen Well Service ran 117 joints of 2-3/8" tubing and two pup joints with a Weatherford Arrow Lockset packer (3692') back in this injection well. Five gallons of inhibitor was mixed with 80 bbl of fresh water. 60 bbl of this mixture was pumped down the annulus between the tubing and the 4-1/2" production casing. The packer was then set in compression. The MIT was completed with an initial pressure of 1015 psi, five minute pressure of 1015 psi, ten minute pressure of 1015 psi and fifteen minute pressure of 1005 psi. After passing the MIT, the annular pressure was manually lowered to 200 psi. The rig was released and moved off location. On 7/9, the annular pressure was 0 psi and the well was on vacuum. As Pander starting injecting, the annular pressure increased to 300 psi and the injection pressure increased to approximately 800 psi. On 7/10, both the annulus and the tubing were on vacuum. Operations were terminated pending evaluation. On 7/14, Moore Well Service rig was on site to correct the pressure leak. While tightening the tubing string by rotation, the packer unseated and another MIT was completed. The initial pressure was 1020 psi, five minute pressure of 1015 psi, ten minute pressure of 1015 psi and fifteen minute pressure of 1015 psi. The second MIT passed.
