-464866713 2013-10-15 I inspected this well to follow up on Chief's Order #2013-136 issued on 9/25/2013. The operator, Stephen J. Svetlak, LLC, has failed to maintain proof of liability insuarnce as required by ORC 1509.07(A) and has been found in matierial and substatial violation for failing to maintain coverage. My inspection found that the well was still in production and is in direct violation of the Chief's Order to immediately suspend all oil and gas operations in the State of Ohio and 1509.07[C] of the Ohio Revised Code. I documented that the well is producing into a Dominion/East Ohio Gas line and took pictures inside the meter run showing production and that the chart was changed out by Tri State Measurement on 10/01/13. The operator has not reported production since 2009.
1342547596 2012-07-17 Follow up to a complaint logged through Department of Natural Resources mail by a landowner about an idle well on her property. Further research found that the well is not on her property but holds the mineral rights on her property through a 118 acre unitization that was executed in 1965 by previous landowners. The last reported production from this well was in 2009. I inspected the meter run and found that the production chart was changed on 7/10/12 and was recording production. If Mr. Svetlak is producing this well as the meter chart would indicate, then he is not reporting production as required. The well and tank battery also do not have the required identification. Both issues need to be addressed immediately. 9-9-05(A)10

2145116102 2013-04-04 Follow up to Compliance Notice #1342547596. I inspected this well and found that nothing has changed from the previous inspection. The grass in the immediate area had been cut down and it was apparent that the well had recently been swabbed to the tank. The wellhead was oily and there was a 10' streak of oil on the grass southeast of the wellhead. There proper ID sign has not been placed on the tank or the wellhead. A minimal amount of oil had dripped from the valve on the stock tank. There was a domestic hook-up on the well with 6psi. I checked the meter run and found that the chart had been changed on 4/2/2012 and indicated that the well was producing. The operator has not reported production since 2009. This violation continues.
