On site to witness shooting off of casing casing was shoot off @3240',when rig crew hooked on casing it was parted@642' pulled pipe out awaiting orders from company office,company rep stated they would resume working on well 5/31/05 attempting to fish casing out of hole if unable to retrive pipe operator will finish plugging well,as of now operator is planning on producing Berea/Gordon sands if they can retrive pipe.

Stopped by service rig for update on completing plug-back operation,they were running 4.5 casing in well,ran2155' of new9.5 casing and cemented with 150 sk. Cement which should cover800' on backside of casing.

Inspection of the well head found the following equipment -4.5 production casing -2.375 tubing & rods with the well equipped with a pumping unit and the production valves open . A 100 barrel tank -separator and gas meter are present in an established dike with proper well identification present .

My inspection today finds the well is equipped with 4.5" casing, 2.375", tubing, rods, and a pump jack. The tank battery consists of one 100 bbl tank, a meter, and a separator. The tank is contained in a dike and has proper identification.

Stopped by service rig for update on fishing job rig operator stated they had pipe on overshot did'nt know for sure if they retrieved pipe where they had shot off or not,what casing they had on trailer was in bad shape holes etc..They had originally shot it off@3230' when they got out of hole they had recovered 2730' of casing.will set rip plug later on in week operator stated they would set cement plug @2600-2400' and then log hole to determine what depth they would run casing in hole to produce berea/gordon sands.
