701503643 2012-07-13 I witnessed the plugging and abandonment of this well with Mr. Kip Thompson today. The wellbore location was verified with a GPS and recorded on this inspection. Mr. Kenny Wright's (Wright Well Service) service rig was on location and swabbed well dry prior to plugging. The well was filled solid from total depth to surface with approx 1 yard of 9 sack grout inside 3.5" casing. Note: When this was drilled, the 3.5" casing (annulus) had been cemented to the surface therefore, no casing was retrieved. Instructed company representatives to leave well bore open over the weekend to allow for potential settling and check for leakage. The previously noted violation(s) have now been resolved.
