-1715752944 2013-12-17 I performed a status check on the City of Akron # 78 ( SMP #4 ) and found that the Dallas Morris drilling rig had shut down at 1138' KB. Their Mud Pump engine blew-up and were awaiting a replacement. Not drilling ahead, shutdown.No violations

1386606583 2013-12-06 Dallas Morris Drilling Rig #15 moving on location and rigging. Conducted prespud meeting with Mike Schumacher ( 586.530.8111 ) of Cargill Inc. to discuss casing hardware, wait on cement ( WOC ) time, Inspector notication, corrective measures if the horizontal lateral from Well #77 to Well #76 is encountered. No violations found at this time.
1387469764 2013-12-18 I checked the drilling progress on the City Of Akron # 78 well being drilled by Dallas Morris Rig # 15. At this inspection the rig was drilling at 1416' KB with a rate of penetration of 16.8 '/hr. The rig is using a Fresh Water mud system at this time. No violations were identified at this inspection
1387557582 2013-12-29 I inspected the drilling of the City of Akron #78 ( SMP # 4 ) @ 0945 hrs. Presently, drill string is hung in hole. Bit is at 2040 KB. Mud pump problems leading to dirty hole conditions and hung bit and sub. Scheduled to switch to brine fluid drilling system today. I found Hydralic fluid on the ground around the Mud pumps and instructed the rig crew to lay down absorbent diapers to remove the fluid. The spill was cleaned -up satisfactory.
1388409095 2013-12-29 I was on this location to inspect the placement and cementing of the 7" casing string. Dallas Morris Rig #15 ran 69 joints of 7" , 23# , J-55 , ERW, R-3 , ST&C , 8rd. Casing. Total run-in tally -3046' GL. Casing hardware included Float Guide shoe, 12 centralizers ( ran on every 6th collar ). Universal Well Services cemented with 830 sacks of Type 1 w/ 5lbs per sack Gilsonite, 3% NaCl , 1/4% ( Vol. ) foamcheck. Mix water was saturated Brine ( 10.1 ). I observed 35 barrels of cement circulated to the steel pits. No violations were identified .
1391095261 2014-01-30 I was on location to observe the Cement Bond Evaluation and pressure test of the 7" casing. Baker Hughes was providing the logging services and Hilco Treating Services was providing the pressure testing services. Hilco top off the 7" casing with Brine then tied into the casing ( through a 2" port ) to catch any fluid displaced by the logging tools and wireline. Permit Construction Conditions required a pressure test to a minimum of 525 psi held for a 15 minute duration. Hilco pressured up on the 7" casing to 600 psi and isolated the 7" casing from the pump truck. Air was bled out of the system repeatedly and @1349 hrs. Hilco pressured up to 600 psi to start the test.. At 1354 hrs. -600 psi. , @ 1359 hrs. - 593 psi. , @ 1404 hrs. - 590 psi. , test ended @ 1408 hrs. with a final pressure reading of 590 psi. A 10 psi drop is within the 5 % allowable ( 30 psi ) limits. Test passed
419189501 2013-12-08 I was on location to witness the cementing of the Conductor casing. Dallas Morris Rig #15 drove 24" casing to 7'. The Rig drilled 20" hole to 120' and ran 16" casing to 102'. Universal Well Services moved in and cemented the 16" casing with 140 sacks of Type 1, 3 % CaCl2 and displaced to 82' with 18.5 Bbls. fresh water. I observed approximately 11 Bbls of cement returns to the half round. Displacement pressure was 178 psi. Water temp. was 42 degrees and pH was 7. No violations were identied at this inspection.
