-639936291 2009-04-23 I met with Steve Clark (330-701-7852) at 4000 Beechwood Ave., Alliance, OH, regarding the new charcoal filter that has been placed on the stock tank at the tank battery that is located west of his home. He said that the odor has decreased and his dogs are no longer getting sick. However, Steve has been diagnosed with leukemia resulting from benzene poisoning. I suggested that he call the Stark County Health Dept. in regard to his medical condition. I called Greg New and informed him of Steve Clark's condition.
1238527368 2009-03-31 I received a complaint call on 3-30-2009 from Steve Clark (330-701-7852) at 4000 Beechwood Ave., Alliance, OH, regarding a strong gas odor coming from the tank battery that is located west of his home. He said that gas keeps blowing from the separator and that he has been sick for 10 months and is getting checked for methane poisoning. I called Greg New who is taking care of this well for Cathy Lesher (C & T Energy) and discussed the complaint. On 3-31-2009, I inspected the tank battery. There was no gas leak observed at the separator. Gas was observed venting from the 2 dia vent pipe located on the oil stock tank. I called Greg New and requested that he place a charcoal filter on the vent pipe. I called Steve Clark and informed him of my inspection and recommendations.
1240235115 2009-04-17 See Complaint #09-001. The charcoal filter has been placed on the stock tank. The complaint has been RESOLVED.
1242505406 2009-05-15 I received a complaint call at 0845 hrs from Steve Clark (330-701-7852) at 4000 Beechwood Ave., Alliance, OH, regarding the odor at the stock tank at the tank battery that is located west of his home. He said that the odor has increased and that he has a headache and his dogs are getting sick. I called Greg New and requested that he send the pumper out to check the separator and the charcoal filter. Greg called me back and said that the separator dump valve was stuck open and that the charcoal filter was plugged. The problem was resolved. I called Steve Clark at 1120 hrs and informed him that the problem was resolved. Steve was at the hospital getting checked for his health concerns.
