-1297478237 2011-08-24 Received telephone call from the well owner's pumper, Mr. Bobby Kerrigan. Mr. Kerrigan was instructed to remove all oilfield debris and pollution and contamination. If the existing sales is not going to be used, it will need to be removed; and repaired if remains in operation.
1218728462 2008-07-25 Jeff Fry and I inspected the well location and met with landowners Tim and Joan Warner at their request. John Loveridge has a plugging permit for this well. There is crude oil on the ground surface within 100 feet of the well. This appears to be a line leak. Also, there is another location in the horse pasture field where crude oil is emerging to the surface. This area has been disturbed by digging. This is a possible well that is leaking to the surface. We will contact John Loveridge and further investigate.
1218734088 2008-08-13 Jeff Fry and I met with John Loveridge and the property owners Time and Joan Warner at the well site. John has a plugging permit that is valid through October and plans to plug the well. At that time he will clean up oil from the line leak. The oil that is in the horse pasture field is not associated with John's wells. Tim was digging in the field with a back hoe when the oil appeared. He said that he didn't hit any lines or well casing. He will clean up the oil and dig again to see if he can find an old well casing or bore hole. The well that is leaking could possibly be 60972. Tim will call if he finds anything.
1315340519 2011-08-10 An inspection revealed that the Final Restoration does not meet ORC 1509.072. The landowner, Mr. Tim Warner was present for the inspection. Mr. Warner states that oil recently flowed from the observed above ground 5" casing with 1" and valve. Mr. Warner further states that he remembers well being plugged and feels it may be leaking. RBDMS shows well location near this casing as plugged and abandoned. No leakage observed at time of inspection however, oil stained ground extends from this location approx 50' in length x 5'. The casing location would be determined as flowline not wellbore related. Oilfield debris consisting of steel line and electric box were also observed at the well site. NONE3
1326910136 2011-11-30 A follow-up inspection revealed compliance. The electric will not be removed, as it continues to provide power to a well on this lease. Final Restoration meets ORC 1509.072.
1920296493 2011-08-17 An inspection of the well site was conducted with Will Ziegler and Randy Moran (Plugging Contractor). During the inspection, it was confirmed that the above ground 5" casing relates to a lead line and not wellbore. The well owner will be notified of inspection results and restoration requirements.
416699865 2011-08-15 An inspection of this location was conducted with Will Ziegler to verify 5" casing containing 1" gas line with valve. High Range Quantabs placed in fluid near this location was stopped at 943 PPM (mg/L) Cl-. The plugging contractor, Mr. Randy Moran, will be contacted to verify plugged well location.
899155186 2009-01-08 Jeff Fry and I met with the landowner Tim Warner at his request. He was concerned with the restoration efforts of this recently plugged well. There is a small amount of free standing crude oil in his pasture that has possibly come from an old lead line. He advised that he will use his backhoe to clean up this mess himself as well as remove the lead line.
