11279392 2002-06-17 Received a phone call from Bath Township Fire reporting a well fire at the Gentsch Unit #1 well head on June 15. The ignition source appeared to be a small natural gas compressor adjacent to the well head. Bath Fire shut the well in, extinguished the fire, bled the lines and contacted Windsor's pumper, Joe Pekarak (330-607-3333). This inspection indicated that the damage was limited to the compressor and a portion of the chain link fence surrounding the well head. The heat may have also affected the well head connections and seals indicated by small oil leaks. All need to be tested prior to resuming production. The impacted material around the well head needs to be removed and properly disposed. The tank battery was not properly diked and the sign on the 210 stock tank indicated the wrong company and emergency phone numbers. The tank battery needs to be properly diked and the old sign must be removed or painted over.

2155i120 2002-06-26 Follow up inspection of the Gentsch well site with Supr. Cheslock. Windsor has not yet begun to clean up the fire damaged areas. Met with and obtained photographs from the Bath Fire Department. The ignition source is still believed to be the electric motor on the gas compressor. Also spoke with Windsor's pumper Joe Pekarak (330-607-3333). He indicated that there intention was to repair the damage and to place the well back into production. He was informed that MRM may not permit the compressor placement so close to the wellhead. Also requested that MRM be notified when Windsor begins clean-up work. Called the property owners, Sue and Dennis Kunas (330-666-2805) and left a message with an update of activities.
2155i139 2002-07-16 Follow up to NOV issued after a compressor fire adjacent to the well head. The compressor debris has been removed and the well head has been rebuilt. The tank battery dike and the signage issues still need to be addressed.
