-209676319 2007-09-17 Site has been ruff graded. Sloped to the pond and the road are greater than 3:1. The slopes have not been seeded, not drainage controls are in place. Sedimentation is evident in the pond from the site. Site needs mulched, seeded and graded. Lease road needs restored. Front of lease road has been rocked, 80'. Remanded of the lease road needs to be constructed out of durable material and stabilized. Supervisor Jay Cheslock had requested that Hall and Horning complete reclamation on the site earlier than 9 months. Site is on the side of a hill. Site needs mulched, seeded and vegetation established. Offsite sedimentation needs controlled. NONEC
1181226824 2007-06-07 Well head has been installed. One frac tank on site. Silt fence is up along the lease road. Site needs to be reclaimed as soon as possible. Spoke to Randy Hall regarding reclamation of the site. He will be moving on to this site next. Site is on the side of a hill and needs seeded to prevent erosion.
1325459118 2007-02-12 On site for conductor cement. 14.75" hole 63 feet deep, 60 feet of 11.75 feet of conductor set. 1 bbl return to surface. Cement held at surface. Plug down at 11:00 pm. Driller called earlier in the day, informed me that he misplaced his permit, and that he had called Randy Hall to request a new copy. He had not received a return call from Randy hall. I read him the permit conditions over the phone. Once on site I gave the tool pusher my copy of the permit. Informed him to have one on site before he nipples up. Called Randy Hall informed him that he will be receiving an NOV, for failure to have a permit on site. NONEC
1874069977 2008-08-13 On site with Dan Horning and Ed Schultz. Lease road is stable to the top of the hill and constructed out of durable material. Top of the lease road has rill erosion forming that needs repaired. A drainage channel may need to be constructed on the west side of the lease road. Drain pipe that runs under the lease road at the top of the hill is causing gully erosion, to the east of the lease road. A new pipe will need to be installed and discharge will need to be at the bottom of the hill to prevent erosion. When clearing site operator pushed soil into the trees, south east corner of affected area. Operator will need to pull soil from out of trees, south east corner of affected area. This dirt can be used to stabilize hill side along the lease road. Hill side adjacent to the lease has gully damage from the drain pipe as well as rill erosion in areas. All erosion damage needs repaired and seeded.
2015797942 2013-07-25 My inspection found the well to be equipped with a plunger lift unit. The well and tank battery are in good repair. The tank battery consists of a 210 BBL production tank, a 25 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
805152226 2008-08-05 Lease road is constructed out of durable material. Well head and tank battery spacing is good. Slope east of lease road needs stabilized, there is rill and gully erosion forming. Drain pipe needs repaired, discharge is causing erosion. Dirt needs pulled out from around trees, dirt can be used to repair erosion.
